We would kindly request guests to submit their information to our guest register.
Please download the “Guest Register” below, fill it out, and send the email which the file is attached to info@cocon-setouchi.com or bring it to Setouchi Jam's Garden Jam's Boutique (〒742-2804 331-8 Hinomichi, Suo-Oshima Town, Oshima District, Yamaguchi Prefecture) near Cocon Setouchi by checkout time.
Alternatively, a similar guest register is available in your room's living area. Please fill it out and bring it to the above location.
For the PDF version, please download, print, and fill it out by hand.
For the Excel version, please download the file via “File” > “Download” and enter your information directly.
We also ask that guests staying at “Villa WithDogs” with their dogs fill out the “Agreement on Dog Usage Rules” in addition to the guest list and bring it to Setouchi Jam's Garden Jam's Boutique along with the guest list.
The “Agreement on Dog Use Policy” is also available in the living room of your room, so if it is difficult to prepare in advance, please fill it out and bring it with you.
